Requirements

A nomination must be submitted for the consideration to the judging panel. All nominations should consist of the following:

• Statements and supporting examples/case studies that specifically address the criteria. This section shall be no more than 1,000 words.
• A 200-word statement summarizing why the applicant would be a worthy recipient of the award.
• Names and contact details of two references who may be contacted to support the award nomination.
• Name and contact details for the person completing the nomination.

You may fill out and send the nomination online or you may send a Word file with all the nomination data (download it here). Nominations must be received no later than midnight, February 3, 2012. Any nominations received after midnight will be ineligible.

For all categories, the qualifying project completion period is January 1, 2011 to December 31, 2011.
